NEW PATIENTS:
Please call (480)-912-6214 to speak to our staff. After gathering some information we will send you an email invitation to our secure patient portal where you will be able to create a user name and password, complete your patient registration, and health history prior to your visit.
EXISTING PATIENTS:
If you are already a patient, you can login to our secure patient portal below at anytime to request an appointment and view medical records.
TELEMEDICINE-VISITS:
We can now schedule telemedicine-visits for sick patient. Please contact your insurance company to make sure they will pay for these visits prior to requesting a visit. You will be required to acknowledge the costs and your responsibility for said costs, regardless of your insurance. Telemedicine-visits are for current patients only. Please call the office for further information.
APPOINTMENT CHECK-IN:
Prior to each appointment please login below and check-in for your appointment. You will be asked to complete a brief questionnaire related to the reason for your visit and can also update any changes to your information at that time.
